To get started
  • Initial Consultation - Speak directly with a Trinity Quality Homecare professional so that we can get to know you and your needs.
  • In-Home Assessment - Schedule a visit by our Registered Nurse for a full evaluation of physical, emotional and relational needs of the prospective client, and a safety assessment of the home.
  • Develop the Plan of Care - Our Registered Nurse develops a Plan of Care designed to meet the identified needs of the client.
  • Care Delivery Agreement - We develop an agreement that implements the Plan of Care and governs the services provided, rates and start date.
  • Caregiver Assigned - we assign a Caregiver to the client, based on the needs assessment, matching the skills of the Caregiver to the needs of the client.
  • First-Day Introduction - We return to your home on the first day of service, introduce the Caregiver, and orient the client and/or family and the Caregiver to the Plan of Care. We want everyone to have the same high expectations of a trusting, professional, personal service relationship!
  • On-going Case Review - A Trinity Quality Homecare supervisor monitors our services routinely, both by telephone and in person, to ensure that we are constantly providing the highest possible quality of service, and to identify potential changes in need.
  • Plan of Care Review - Over time, the needs of the client may change. When such a need is identified, from any source (client, family, family doctor or advisor, Caregiver, or supervisor review), a Trinity Quality Homecare Registered Nurse will visit our client, review needs, and modify the Plan of Care if required.
